Contracts For Participation
CONTRACT FOR PARTICIPATION IN FOOTBALL/SOCCER ACTIVITY
Acknowledgment of Risks: Participant acknowledges that playing football/soccer involves risks of bodily injury, including but not limited to [insert specific risks], and hereby assumes those risks.
Assumption of Risks: Participant assumes all risks associated with playing football/soccer, including those resulting from the negligence of Organizer, venue or other participants.
Release of Liability: Participant releases Organizer, venue and all other participants from any and all claims, actions, damages, or liability arising from or related to Participant's participation in football/soccer, including any injuries or damages that may result.
Covenant Not to Sue: Participant agrees not to sue Organizer, venue or any other participants for any claims, actions, damages, or liability arising from or related to Participant's participation in football/soccer.
Indemnification: Participant agrees to indemnify and hold harmless Organizer, venue and all other participants from any and all claims, actions, damages, or liability arising from or related to Participant's participation in football/soccer.
Medical Treatment: Participant authorizes Organizer to obtain medical treatment for Participant in the event of an emergency.
Waiver of Right to Trial: Participant waives any right to a trial by jury in any legal proceeding arising from or related to Participant's participation in football/soccer.
Photo and Video Release: Participant authorizes Organizer to use Participant's name, likeness, and any photos or videos taken of Participant during the football/soccer activity for promotional or other purposes.
Severability: If any provision of this Contract is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
Governing Law: This Contract shall be governed by and construed in accordance with the laws of Australia.
The Participant represents and warrants that they are over the age of 18 and have the legal capacity to enter into this agreement. The Participant acknowledges that they have read and fully understand the terms of this waiver form and agree to be bound by its terms.
If required the Participant represents and warrants that they have obtained the necessary consent from their parent or legal guardian to participate in the football/soccer activity. The Participant acknowledges that they have read and fully understand the terms of this waiver form and agree to be bound by its terms.
